Attached is a questionnaire that
you should fill out at your earliest possible convenience (Please return by
**/**/**).
The purpose of this questionnaire
is to aid us in developing a workable Business Recovery Plan. As you proceed,
please keep in mind that the chances of a disaster happening here are HIGHLY
REMOTE. If we were to ever find our organization in a crisis mode, the results
of this questionnaire would be very helpful.
Please answer the questions to
the best of your ability. You should fill out one form for each function your
department performs. A function is a macro level process.

Critical Function Questionnaire - Definitions
FUNCTION: A function is a
macro level process which may consist of a number of steps. At this time, we are
not concerned with the steps involved in completing the function. An example of
a function is Accounts Receivable. An example of a step for this function is
"print invoice".
LOSS OF REVENUES: All
organizations need a constant source of revenue to survive. Functions within
the organization that provide the revenue stream must continue in the event of a
disaster. Accuracy is vital to the establishment of these figures; you should
be prepared to justify your answers.
ADDITIONAL COSTS: In addition
to lost revenue, we must also look at which functions within the organization
will result in increased costs if not performed may result in fines, lost
leases, lost discount, cancelled contracts, etc.
HUMAN LIFE IN JEOPARDY: These
are functions that if not performed following a disaster, would put a person's
life at risk.
POTENTIAL TO CAUSE ILL-WILL: These are functions that if not performed following a disaster will
create some form of dissatisfaction between our organization and its employees,
vendors, etc.
IMPACT ON OPERATING
INEFFICIENCIES: These are functions
which directly affect the day to day business of our organization and may
have an impact on other important functions.
IMPACT ON CUSTOMER SERVICE:
A temporary business shutdown due to a disaster might be tolerated by our
customers. However, an extended business shutdown will force our customers to
find another vendor and we may never regain their business. The departments
that provide essential customer service should be included in the plan.
LEGAL REQUIREMENTS NOT MET: A
function, not performed, will cause legal ramifications. Such as law suites and
court.
IMPACT ON ENVIRONMENT:
A function, not performed, will cause harmful effects on the environment. For
example, daily cleaning of medical waste bins. If left undone, would create a
potentially hazardous impact on a Doctor’s office.
